There are two types of Special Permission Forms for the Regular Season:

1. Permission to play in two age groups

This means you are asking to permanently play in two teams throughout the season.  The permission may be granted for a specific period of time, for the whole regular season or the whole season (including finals).

2. Medical Permission

This means you are asking to play in an age-group below your qualified age due to medical reasons.
