REGISTRATION PROCESS
FV Futsal Club Affiliation.
Football Victoria (FV) welcomes new futsal clubs who are interested in affiliating.
The process starts by filling in the New Futsal Club Application Jot Form and the FFA’s NR002 form. Please note that a club must be a legal entity in the first instance and ideally have at least two teams. The expectation on FV clubs is that they participate in the FV State Club Championships, FV tournaments and the FV Premier League

Play Football’s Self Registration Package
This is a Self-registration system where a player pays at the time of registration. The annual registration fee is $30 for juniors and seniors. All players who aspire to play for a State Representative team, must be registered to a FV Futsal Club in the first instance.
This entitles a registered FV Futsal player:
- to play in any affiliated Futsal centre in Victoria
- to play for any affiliated FV Futsal Clubs or Entities, as displayed in the FV Club List.
- to play in any FV Futsal Regional Tournaments
- to play in the FV Futsal State Club Championships
- to trial for any of the FV Futsal State Programs.
In addition to the above:
- It is an annual fee that is for the calendar year, i.e. February 1 to January 31*
- * subject to confirmation by FFA
- There are no pro rata rates for players who register later in the calendar year
- It does not cover a Club's subscription fee, match fees for games in social leagues, etc, or any other fees that a Club or Entity may charge.

Players Changing Clubs Mid - Season
In the instance a player wishes to change Clubs, please ensure an NRR 08 Cancellation of Amateur Registration form and NRR 11 - Supplementary Registration form are submitted to FV for processing. This will ensure that the player is not paying the FFA Insurance Fee twice in one year. Cancellation fees may apply.
