Coaches note: Members Declaration form
This is an annual form for ALL coaches to sign, including Police officers/teachers and parents of players. Submit to HJBA office each year. This is a requirement of Basketball Victoria, as stated in attached information.
Clearance form
To be completed by your 'old' club secretary/president and by parent/guardian prior to handing into the HJBA office.
