Clearance Form
You are required to fill in the below form if you wish to transfer to a different domestic club.
It is the responsibility of the player to lodge the clearance with the Cockburn Basketball Association office.
It is the responsibility of the new club to ensure this clearance has been approved and processed before the player takes the court. It may take up to 7 days for a clearance to be processed.
