Please complete the Player Application form and bring the form on the first date that you attend the 2019 Waves Trials along with payment. $10 for each trial and you must attend both to be consider.
If unable to attend trials for any reason a Special Consideration form must be submitted along with Player Application Form.

The new form includes multiple lines where players will be required to enter ALL the associations they are trialing at. This will assist associations, as they will not be required to complete multiple forms for the same player. Players will be required to use the same form at each trial.

If your child/ren were successfully chosen in a Waves 2019 Representative team these are the forms that need to be filled out and returned with your commitment fee on or before CLUB DAY which will be held at Niagara Park Stadium on 15th September 2018. Times to be advised. Please print out your forms and bring on the day.
