Senior Player Clearance Form

Below is the standard Player clearance form for all Players requiring a clearance from one Club/Team to another. 

Please print the attached form, have if fully completed including the authorised signatures of both the Club/Team you are leaving and the also the Club/Team you are going to. 

Once completed please lodge direct with the BMBA Admin Manager ( Samantha Clayton). 

Note: You will only be able to take the court for your new Club/team only after this form has been duly posted as approved under the clearance section of this Website.
