Team Nomination Forms
Team Nomination Forms are required to be completed if registering an already formed/existing team (Seniors), External Club and/or School Team (Juniors).
You will require a minimum of 6 players per team and must provide your own coach (Junior Teams Only)
Once form has been completed, please scan/email: registrar@uscbc.basketball.net.au or deliver to the office by applicable seasonal cut off date (listed on applicable Senior and/or Junior Team Nomination Form) - for inclusion in the current seasons competition.
Teams will be formed and eligible to compete once each player has registerd/made payment via the Online Registration Form and upon receipt of your Team Nomination Form.
