Interum Report Form
This form is to be completed immediately (or as soon as practicable) after the game where the incedent occured and photographed.
The original copy should be given to the Offender or Club Deligate if Offender is no longer in stadium.
The photo must be sent immediately to the Referee Co-ordinator.
A full Official Basketball Tasmania Report Form must be completed and scanned/photographed and sent to the Referee Co-ordinator within 48 hours of the incedent.
