The Personal Injury Claim Form and the Personal Injury Cover Summary provides information about claimable benefits and How to claim.
Claimants should read through the documents and complete the Claim Form.
It is the responsibility of the claimant to arrange for the claim form to be completed, including the Physicians Statement.
Clubs should ensure they have a record of the injury and the player was registered at the time of the injury before signing the Club Declaration.

Send the completed claim form to Gow-Gates within 120 days from the date of injury via:

The claimant should make a copy of their claim form and keep for their records if posting.
Once a claim has been assessed Insurer will write directly back to the claimant and advise the outcome of the claim.
Receipts for reimbursement of claimable benefits can be sent to Insurer with the Claim Form or at any time after receiving reply from Insurer.
